Congenital heart defects are a significant concern, being the most common and resource-intensive birth defects in the United States. While complex heart defects often require treatment at regional cardiac centers, mild defects can be safely managed at community hospitals.
Dr. Joyce Woo, a pediatric cardiologist at Ann & Robert H. Lurie Children's Hospital of Chicago, emphasizes the importance of prenatal care in guiding appropriate care for newborns with heart defects. She notes that a single prenatal visit may not be sufficient, as fetal heart conditions can evolve during pregnancy, potentially becoming less severe. If surgery is not immediately required, staying closer to home for delivery can be a preferable and beneficial decision for both the baby and the family.
The study, conducted by Dr. Woo and her colleagues, analyzed data from a statewide administrative database, focusing on 12,113 babies born with heart defects in Illinois between 2013 and 2021. They aimed to understand the relationship between the initiation and frequency of prenatal care and the delivery location for neonates with congenital heart defects.
The findings revealed that delayed prenatal care initiation was associated with an increased likelihood of delivery at a cardiac surgical center, regardless of the severity of the heart defect. However, for babies with mild congenital heart defects, more prenatal visits were linked to a significant decrease in the probability of delivery at such centers.
Dr. Woo explains, "Caring for children with heart defects starts before birth. Prenatal care ensures that babies with congenital heart defects are born at the right place, with the appropriate level of care tailored to their specific needs. It also considers the family's preferences for delivery location."
This research not only highlights the importance of prenatal care but also has implications for statewide policies on perinatal resource allocation.
